Position: SEND Case Officer
My Client is seeking a dedicated Case Officer to make a positive impact in people’s lives within the health and social care sector. The council is committed to recruiting and developing talented individuals who align with their values and ambitions.
About You: As a Case Officer, your responsibilities will include:
- Overseeing the customer journey, assessing reablement needs, and developing delivery plans for exit planning.
- Providing line management and guidance to Supervisors and Support Workers.
- Conducting visits to customers to identify reablement needs and developing personal outcome plans based on those needs.
- Undertaking customer reviews, risk assessments, and responding to rapid changes in customers’ skills and confidence.
- Ensuring the delivery of the service complies with Care Quality Commission requirements.
- Demonstrating excellent IT and organizational skills, with confidence in liaising with people at all levels.
About The Role: The reablement service focuses on supporting individuals in their homes, helping them rebuild confidence and independence for safe living. As a Case Officer, you will play a pivotal role in facilitating safe and successful customer journeys towards independence.
