💥 URGENT REQUIREMENT FOR AN ICB CLIENT OF MINE IN THE SOUTHWEST – CHC Specialist Practitioner role 💥

The ICB are looking for an experienced complex care professional to assist them with case management projects for individuals with complex health needs. They need someone who is familiar with the continuing health care process and is experienced working on complex cases.

This position will cover both adults and a CYP portfolio.
